What to Expect

1. Intake Questionnaires: To access this HIPAA-compliant portal, CLICK HERE to arrive at Enroot Wellness within Living Matrix to submit your intake questionnaires.
2. Registration: you will fill in some personal information to get started in a form like this:                                                           
3. Email & Activation code: For privacy purposes, you will be sent an email as part of the dual verification process. Click on the link provided in the email, type in activation code provided, and complete your registration.- All emails come from no-reply+support@livingmatrix.com with
   – Email subject line “Enroot Wellness…”
   – Login is your email and you will create a password.
   HINT: SAVE your password (under Enroot Wellness contact in your phone, for example, so you will always have easy access).
4. Popup: you can choose to watch the video or simply close the message
5. Questionnaire parts: click on little blue arrow next to each questionnaire
   
6. Completion of questionnaires: It is here where you will provide in-depth information about your story, primary health concerns, family history (if applicable), goals, supplements, medications, and more. Please provide as many dates as possible for when symptoms began. These usually take about an hour to complete, however, you don’t have to do them all at once. You can complete part/s of it and come back at a later time to finish. Within your dashboard, those not yet completed will indicate the status “Pending”.
